Pre-arranged absences of three or more days: The pre-arranged absence form for School District 4J is used to request an excused absence for a planned event, such as a family trip. The form is required for absences of three or more days, and must be completed and submitted in advance:
When to complete: At least one week before the absence

Who to get approval from: The principal and the student's teachers.
The form does not apply to emergencies, sickness, or authorized religious holidays. Absences due to illness that last more than five days require a doctor's note. Permission to be absent does not guarantee that the absence will be excused. Vacations are typically not considered excused absences

It’s School Choice Time. See what our schools offer!: In Eugene School District 4J, students may request to enroll in any school. School choice
requests are accepted in an order determined by lottery.
January is school choice season: Virtual school choice visits and in-person school visits are
scheduled in the month of January. 4J residents must submit a request form between January
1 and January 31 to be included in the annual school choice lottery. Non-district residents must
submit a request form for an interdistrict transfer between March 1 and March 31 to be
included in the lottery.
Who needs to apply: All students who wish to enroll in a new school other than their
neighborhood school must submit a school choice request form. This includes:
● incoming siblings of current students.
● students who have moved out of our school neighborhood or the district and want to
remain enrolled next year. Their enrollment is not automatic and not all schools are
open to non-district resident transfer requests.
● fifth grade students who want to enroll in a non-neighborhood middle school.
Who does not need to apply: If your student is already enrolled in our school and has not
moved, you do not need to submit a request form to continue attending our school next year.
See our schools: If you or any of your friends are interested in exploring your school choice
options, all 4J schools will hold virtual visits and in-person info sessions with an opportunity to
ask questions during the month of January. Additionally, the district is holding school choice
information sessions on:

Safe Touch & Erin’s Law: Your child will be receiving lessons around Safe Touch. The lessons are part of our school's health program, and are mandated by the state through Erin’s Law.
School programs can help children learn to stay safe from abuse. The lessons being taught present material at the student's age/development level. The skills needed to prevent abuse are taught in whole group classroom lessons.
Through these lessons, students learn:
How to judge between safe and unsafe touch
How to protect themselves
Who to go to for help

Are you interested in volunteering as a classroom support parent? McCornack’s PTO is looking for parents and guardians to help with:
Assisting teachers in organizing and coordinating classroom events (holiday parties, end-of-year celebrations, etc.).
Gathering donations or supplies for special projects.
Coordinating volunteers for classroom activities like reading helpers, art projects, or field trips.

Family Game Checkout: Want to check out a game from the McCornack/PTO game cabinet? Fill out this FORM and either select checking out “in person” or “send home with student”. Please return games within 2 weeks (with all the pieces please or a note letting us know of a missing piece!)
Lost and Found: We have an abundance of lost items located near the front office. All unclaimed lost items will be donated during winter break. We’d love for all the lost things to find their homes.
Family Resource Corner: My name is Sophia Durant. I would like to introduce myself. I am the Family Resource Coordinator for the families of McCornack. I am here for you in times of need and can provide you guidance and resources. Let me know if you have any questions. I also have office ipads available for families who need assistance in accessing online forms, resources, etc. You can make an appointment with me and I can help you navigate technology.
