2025/2026 Parent and Family Engagement & Partnership Survey:

Thank you for taking a few minutes to complete this survey. Your feedback will be used to inform our efforts to continually improve our district, our schools, and our partnership with parents to help students succeed.

If you have more than one student attending school in Eugene School District 4J and have had a different experience for different students, please fill out a separate survey for each student. That provides the schools with information specific to each student’s individual educational experience.

If you have questions or input regarding this survey, please call the Instruction Department at 541-790-7550. Once again, thank you for your time and input.

Caregivers of 5th grade students:

We are putting together a slideshow for the upcoming 5th grade promotion and we need your help!

Please send us:

  • A baby picture of your 5th grader (aim for the first year of life)

  • A picture of your child from this year (a current photo, school photo, etc)

If you would like to include pictures of your child and their friends throughout the year (or elementary years) you are welcome to also send a few photos that we can include in the slideshow.

We would love to have pictures of all the kids to share! Please send your photos in by June 2nd!

You can email your photos to mccornack5thgrade2026@gmail.com and include the child’s name in the subject line. You can also upload the photo directly here:

https://photos.app.goo.gl/CAvDGE4hkbsBwDWK9

The full slideshow will be shared with everyone after promotion.

McCornack Carnival: Thursday, June 4th from 6:00-8:00pm:

We need YOU to help make this fun family event a huge success. Here are some ways you can support this event and our school.

1. Volunteers: Each classroom will be responsible for a carnival game and will need at least 6 volunteers from the class to take shifts throughout the carnival. And there will be other volunteer opportunities to sign up for as well. A volunteer sign up was sent out via ParentSquare - CLICK HERE to view the sign up. Please find an hour that you can volunteer and you will receive FREE tickets as a thank you for taking time to help our carnival run smoothly.

2. Auction Baskets: Each grade-level is responsible to put together a themed basket for our auction. Please check your child's backpack for a yellow flier describing the theme that you can make donations for. Also, if you have any baskets or bins lying around that you could donate to assemble the items in, then that would be helpful too.

3. Business Donations for the Auction: If you or someone you know has an item or service from a local business that they can donate to our auction, then we will happily accept them. More information about this, including the EIN number, will be going out after spring break.

Carnival Ticket Pre-Sales Are Now Open!

Get ready for an evening full of games, prizes, food, and family fun at the Busy Bees PTO Carnival!

Get your student’s FREE LTD transit pass before the end of the school year!

All students in 4J schools can get a student pass to ride Lane Transit District (LTD) buses for free. Student transit passes can be used to travel anywhere within the LTD service area. Student passes can be used at any time—after school, on weekends and during school breaks, as well as to get to and from school each day. And LTD passes will be $1 per trip in July and August, so you can ride with them for a very low cost.

To get a free LTD student transit pass, available as a tap card or Umo smartphone app, guardians/parents can make the request to Julie Whittlesey @ whittlesey_j@4j.lane.edu

Passes are valid as long as your student is at 4J!

Do You Have Outdated Prescription Glasses around?  Ms. Swift’s daughters attend an annual “Animal Camp” hosted by their Grammy and Papa. Each year, the camp includes a community service project. This year, they are collecting outdated prescription glasses to donate to the Lions Club, where they will be refurbished and distributed to those in need.

You can drop off your outdated prescription glasses at the front office of McCornack Elementary. Thank you for supporting this wonderful cause!
